I'm using "Instant Harmonica - A Method for Diatonic Harmonica" by David McKelvy. It includes a CD. Hal Leonard is the publisher. I've been using it for about 7 months, along with alot of information from this site. I figure I can finish the book in a couple more months.

About ten years ago I started with Gindick's books, and quit because I thought I must just not get it, because I couldn't jam in a couple of weeks. This time I'm not quitting, because I've realized the harmonica is just like any other instrument....it takes time and alot of practice.

I can play a decent 12 bar blues, that brings a smile to my face, and I'm having fun. I think McKelvy's method is great. It works, but don't expect any method to be "instant".
