If you guys are talking about the "two-note-shake," this is how it's done:
While holding your harmonica steady, shake your head from side to side. Do it like you are making a "no" gesture.
The more common shake is probably wile drawing on the 4 and 5 holes (using a 10 hole diatonic), but you can do the "two-hole shake" up and down your harmonica.
This is not difficult, although it may take some practice to get the desired sound and effect.
